SWC Model Release Forms:

 

In general, use this model release for when shooting in the SWC Photo Studio for projects related to Art 207, 210, 211, & 216:

 

SWC Studio Model Release

 

 

Use this model release for when shooting in the SWC Photo Studio with a model from an agency.  Prior to the photo shoot, contact the model and/or agency, to make sure this model release form will be approved by them.  If there is an issue, contact Micajah Truitt or John Pickelle.
